A shield shaped wood and metal plaque, At the top of the plaque is a circular symbol that is divided into four parts, followed by a rose, fluer de lis, clover, and a thistle in each quarter. There is a green leaf at the centre of the symbol. Around the edge of the circle are the words in red lettering that read "Union Progres Cite de Lachine". Below the circular symbol, there is a list of the names of Lachine's city council at the time. The black words are "Jean-Guy Chartier// Maire// Mayor// Leonard E. Baxter// J.C. Brownrigg// Raymond Tessier// Roland Farineau// Victor Timbro// Leo Bourque// conseillers// councillors". The back of the plaque has a small hole on each side and a circular shaped groove at the top. On the upper centre of the back is a triangular metal label that reads "TROPHEE BRODEUR TROPHY// 965 Notre-Dame// Lachine// 637-5901".
