A cream coloured booklet with a blue corded tassel running through the spine. On the front cover is the blue coat of arms for the Province of Ontario, consisting of a Moose, a deer, a shield with maple leaves and a black bear on top of the shield. Below the coat of arms, the title of the booklet in blue lettering reads "Provincial Dinner held on the Occasion of the Presentation of the 1978 Ontario Sports Awards". On the bottom left corner are the blue words "May 11, 1979// Toronto, Ontario". The back cover is plain and in cream colour. Inside this booklet is the list of programming for the night, the menu, and the 1978 Special Achievements Award recipients.
