A shield shaped wooden plaque with a metal crest at the top for the Fort Henry Guard and a brass coloured metal plate on the lower part. The Fort Henry Guard crest is in a star polygon shape with a crown on the top. The initial of the guard "FHG" is inside the circular centre of the crest, and the words around "FHG" read "HONI SOIT QUI MALY PENSE". Below the crest is a metal plate which reads, "Presented to The City of Kingston by The St.Lawrence Parks Commission and Fort Henry on July 14, 1991// In celebration of the 150th Anniversary of the selection of Kingston C.W. as the Capitol of the United Canadas in 1841". There is a groove at the central top on the back of the plaque.
